SUBMISSION AGREEMENT FOR CARROLL SCHOLARS By signing and submitting this license, you (the author(s) or copyright owner) grant to Carroll College the non-exclusive right to reproduce, translate (as defined below), and/or distribute your submission (“the Submission”) worldwide in print and electronic format and in any medium, including but not limited to audio or video. You understand that once a Work is deposited in Carroll Scholars, it will be made publicly accessible. You agree that Carroll College may, without changing the content, translate the Submission to any medium or format for the purpose of preservation. You also agree that Carroll College may keep more than one copy of the Submission for purposes of security, back-up and preservation. You represent that the Submission is your original work, and that you have the right to grant the rights contained in this license. You also represent that the Submission does not, to the best of your knowledge, infringe upon anyone's copyright, nor violate any proprietary rights. You represent that the Submission does not contain any libelous matter, nor invade the privacy of any person or third party, nor contain records protected by state privacy laws. You also represent that the Submission does not contain HIPAA-regulated information (protected health information), except only if the individual has authorized release of his or her information in writing, and such release is in the hands of the publisher. If the Submission contains material for which you do not hold copyright, you represent that you have obtained the unrestricted permission of the copyright owner to grant Carroll College the rights required by this license, and that such third-party owned material is clearly identified and acknowledged within the text or content of the Submission. IF THE SUBMISSION IS BASED UPON WORK THAT HAS BEEN SPONSORED OR SUPPORTED BY AN AGENCY OR ORGANIZATION OTHER THAN Carroll College, YOU REPRESENT THAT YOU HAVE FULFILLED ANY RIGHT OF REVIEW OR OTHER OBLIGATIONS REQUIRED BY SUCH CONTRACT OR AGREEMENT. Carroll College will clearly identify your name(s) as the author(s) or owner(s) of the submission, and will not make any alteration, other than as allowed by this license, to your submission. NOTICE OF SUBMITTED WORK AS POTENTIALLY CONSTITUTING AN EDUCATIONAL RECORD UNDER FERPA: Under FERPA (20 U.S.C. § 1232 g), the Submission may constitute an educational record. You acknowledge this fact and expressly consent to the use of the Submission according to the terms of this agreement. You further understand that you are waiving some of your rights to anonymity under the Family Education Rights and Privacy Act of 1974 (FERPA) and are granting Carroll College the right to display your name in connection with any contribution of which you are an author or co-author.
